Pet owners who purchase animals from HTI Certified Businesses can report cases of illness or death.

To report a case of illness or death (within one week of purchase) of an animal purchased from an HTI business and suspected to have disease, please submit your information below.

Please provide any additional details that may help us. This can include information regarding your husbandry (i.e. tank humidity, temperatures, substrates, diet), the purchase date of the animal(s), or what signs of disease you are observing in your pet(s).
